Yes, the checkboxes represent real-time data that is pulled from the same database records that I use to keep track of the responses.

If you want to give me the details, I can make a small flyer to include with the response card mailing. They could then return both and we would have an idea who will be attending each event.
Alternately, I can print envelopes for a separate mailing if you would rather do it that way.
